This is my final project for the Carnegie Mellon Heinz School of Public Management's Multimedia class, 90-782, Spring 2000 term. I am concerned soley with the form, not the content of survey. While I will tabulate the total responses for each question, how I display the results is important, vice the results themselves. I cannot keep records on individual responses as all responses will come to me anonymously.

The results will be available by May 8, 2000.
